Signal summary

Survodutide is a dual glucagon/GLP-1 receptor agonist being developed for obesity and metabolic liver disease. Its glucagon component makes energy expenditure and fat metabolism particularly relevant to the RecompSignal lens.

Signal interpretation: Survodutide has matured beyond an early pipeline story: Phase 3 obesity trials are completed or completing, and a separate mechanistic study directly compares metabolic effects with semaglutide. An MRI substudy has reported composition results. Full appraisal remains pending; measured lean-volume change does not establish muscle preservation or functional benefit.

Study details

Doses studied — not a dosing recommendation. Regimens belong to the named study, formulation and population.

SYNCHRONIZE-1 phase 3 trial

Population
725 randomized adults with obesity/overweight and complications, excluding diabetes.
Doses and administration studied
Subcutaneous treatment adjusted up to 3.6 or 6.0 mg once weekly versus placebo.
Duration
76 weeks
Reported finding / status
Treatment-regimen mean weight changes: -12.2% and -13.0%, versus -5.4% placebo.
Interpretation and limitations
Gastrointestinal symptoms were frequent. Weight results do not establish lean-mass or functional benefit; completed CVOT follow-up is not proof of cardiovascular benefit.

SYNCHRONIZE-2: adults with type 2 diabetes

The 2026 phase 3 publication randomized 752 adults with type 2 diabetes and BMI at least 27. At 76 weeks, treatment-regimen mean weight changes were -8.2% and -9.8% for the 3.6 and 6.0 mg weekly trial arms, versus -3.9% placebo. These research doses are not a dosing recommendation. Keep this population separate from SYNCHRONIZE-1.

Reported MRI composition analysis

The primary SYNCHRONIZE-1 article reports MRI outcomes in a selected 25-per-arm subset with paired scans and uninterrupted treatment. Fat and lean remain NR pending corrected-version and supplementary-method appraisal. A regression-based lean-loss ratio is not an individual percentage of muscle lost. Scan findings do not establish strength or a muscle-preservation advantage.

Weight: published phase 3 RCT. Fat and lean: NR pending full appraisal of reported MRI results. NR means not rated; it is distinct from 0/5. Function benefit is not established in the selected evidence.

Editorial evidence ratings for the cited outcomes, not effect size or treatment recommendations. A lean-mass score may describe loss. 0 means not established in the reviewed evidence, not proof of no effect. Read the rubric.

What is actually established?

  • SYNCHRONIZE-1 and SYNCHRONIZE-2 have published phase 3 weight results in populations without and with type 2 diabetes, respectively.
  • MRI composition findings have been reported; detailed appraisal is pending.
  • The large Phase 3 cardiovascular outcomes trial enrolled more than 5,500 participants and completed follow-up in 2026.
  • A dedicated study is comparing survodutide with semaglutide on energy use and fat metabolism.

What is not established?

  • Survodutide remains investigational.
  • Metabolic mechanism does not automatically translate into superior body-composition outcomes.
  • Detailed lean-mass and function comparisons remain less mature than weight-loss evidence.
